WHAT IS THE IDIL DISSECTOR?
Cosmetic surgery procedures with calf implants used to be a very difficult procedure. It took nearly 5-6 hours to place these implants intramuscularly in the calf muscle (gastrocnemius) through a small incision posterior to the knee. Moreover, implant manufacturing companies were not offering a favorite solution for this problem. Even different companies have suggested in training videos using a long forceps or a liposuction cannula when installing these implants. Manufacturers usually offer a small pocket at one end of the implants. They recommend to attach a slim, long surgical instrument into this pocket and to push the implant into the lodge where it will be inserted making use of this instrument.
We, too, have tried such solutions. Such slim and long surgical instruments are hardly suitable solutions for installing implants. In fact none of the companies describes you how to open the lodge in which the implants will be placed. They only describe the instructions to insert the implant. They recommend slim and long surgical instruments such as a liposuction cannula to be placed in the pocket of the implant and to be used for inserting the implant; however such slim and long surgical instrument may easily prick into and pierce the implant. The result is: a damaged implant...
All these problems caused us to design a specific surgical instrument. After trying a few different tools, we managed to design a surgical instrument that really met our needs and fixed all these problems we were experiencing; after using it for 10 years, we achieved to patent it We call this surgical instrument “İDİL DISSECTOR”. There are 2 main functions of this instrument:
1) Using the round end of the dissector, we open a space/lodge with sufficient depth to insert the implant: In fact none of the implant manufacturing companies describes you how to open the lodge in which the implants will be placed. I've never come across anything like this in an implant manufacturing company's training video. You will definitely need a long surgical instrument to place the implant intramuscularly, not under the muscular membrane. No surgical instrument, specific for this procedure, had been designed previously. As one end of the idil dissector is round, you can easily push it forward between the muscle fibers and open the lodge where the implant will be placed. As the length of the dissector is 32-33 centimeters, you can easily open a lodge up to +2 cm longer than the implant. In order for the implant to fully fit into the place, the lodge should be 2 centimeters deeper than the length of the implant. A ruler is available on the dissector, starting from the rounded end. So you can easily check how many centimeters deeper you have inserted. If the implant is 17 centimeters long, it should be inserted at a depth of 19 centimeters. As this end of the dissector is round, it will not damage the muscle when you push it forward intramuscularly; it will easily separate the tissues between muscle fibers and will allow you to atraumatically open the lodge where the implant will be placed without any bleeding.
2) Using the protruding end of the dissector, we insert the implant: Many brands offer a pocket at one end of the implant. They recommend to attach a liposuction cannula into this pocket and to push the implant into the lodge where it will be inserted making use of this instrument (similar to putting the bread dough in the oven using a baker’s peel in the bakery shops). However, when you attach a slim and long surgical instrument such as a liposuction cannula in the pocket of the implant and use it for inserting the implant, they may easily prick into and pierce the implant. For this reason the rest of the fine-tipped instrument to be attached in this pocket should be thicker. If you attach the small protrusion at the end of the idil dissector into the pocket of the implant and push the implant forward, the implant will not be damaged as the rest of the instrument is 2 centimeters thick and will not prick into the implant. The implant will be easily "pushed forward" intramuscularly, as the lodge inside has been expanded sufficiently.
Accordingly; with an idil dissector:
- the surgery is completed without any bleeding;
- the implant is placed exactly in the desired place and at a sufficient depth;
- the surgery is finalized in 45 minutes (both legs)
- there is no risk of piercing the implant when inserting it.
This surgical instrument, which was introduced to the plastic surgery community with my presentation at the Aesthetics Surgery Symposium in 2016, have currently been produced and supplied by Elektron Medikal to ordering surgeons. We have been video recording all our surgeries since 2011. Since we started using the idil dissector, we have been able to finalize our aesthetic calf restoration surgeries within 1 hour. We retain the video recording of all these surgeries in our archive.
Thanks to the patented "İdil dissector", it takes an average of 8-10 minutes to place an implant in a leg. The surgery of both legs is finalized within a total of 30 minutes. We record all our surgeries with GoPro. Here, I ask my nurse the duration after finalizing the surgery on the first leg. The answer is: “Eight minutes!” It took only eight minutes to finalize the surgery on the first leg. The entire surgery (from the beginning till the end) takes around 30 minutes.

It’s 8 minutes under normal conditions. When we completed the surgical intervention on the other leg and finalized the operation, the camera's time counter was at 35 minutes. It is the patented “idil calf dissector" of my own design that makes this surgery so practical. Thanks to this instrument, the surgery is completed quickly and easily without any bleeding. It is possible to place the implant exactly to the desired location (intramuscular).

This is a 5-second clip from the 11th minute of another aesthetic calf restoration surgery. It took 8 minutes for us to place the implant in the patient's right leg. When we completed the surgical intervention on the other leg and finalized the operation, the camera's time counter was at 42 minutes. In this example, total duration of the operation is slightly longer (normally, this procedure ends in 30-35 minutes from the beginning to the end) as a surgeon colleague visited us to watch the operation. The operation took a little longer because I was describing my colleague the procedure simultaneously as I performed the surgery. It is the patented “idil calf dissector" of my own design that makes this surgery so practical. Thanks to this instrument, the surgery is completed quickly and easily without any bleeding. It is possible to place the implant exactly to the desired location (intramuscular). "İdil leg dissector" has become a complete milestone and a turning point for this type of surgery. Colleagues, who want to buy this dissector, may call Elektron Medical. I have never expected any financial gain from Elektron Medikal in exchange for the right to use the patent. I just wished this instrument to be sold at an affordable price without the expectation of any profit. I wish this groundbreaking instrument in cosmetic calf surgery to be useful for all plastic surgeons in our country. It facilitated the procedure. It may also be used for other silicone surgeries (breast, butt, arm).

Colleagues, who want to buy this dissector, may call Elektron Medical. The patent rights of the idil dissector belongs exclusively to me. I transferred the rights with regard to manufacturing it to Elektron Medikal without expecting any financial return (I personally assumed all costs associated with the patent process). I do not earn any returns over its sales. I just wished this instrument to be sold at an affordable price without the expectation of any profit. This surgical instrument, which makes aesthetic calf restoration surgery using a calf implant extremely easy, is the first in the world and is our gift to the entire plastic surgery community. My only wish from my surgeon colleagues is to mention the original name as “idil dissector” when they ask their nurses to hand in the instrument during the surgery.
