Silicone Calf Implants
On this page you will find detailed information with regard to silicone calf implants. Silicone calf implants are, in many aspects, similar to silicone breast implants. Silicone gel-containing implants, just like breast implants, contain cohesive, that is high density-consistency silicone gel.
In fact, there are two types of these implants: 'high density silicone gel-containing implants’ and ‘solid silicone implants comprising a silicone elastomer (similar to erasers)’. I will first talk about silicone gel-containing implants, as I prefer to use them for my cases. I'll talk about silicone elastomer implants later..
The gel in the silicone gel-containing implants does not have a liquid consistency, they have the similar consistency with that of soft candies (jellies). As these implants are prepared to be inserted in the calf muscle (gastrocnemius), their shape is also fusiform (elliptical), similar to that of the muscle. Like breast implants, calf implants also have asymmetric (anatomical) and symmetrical models.
The implant to the upper part of the image below is asymmetrical, that is one end is thicker than the other. I try not to use these implants for my female patients as much as possible, because they tend to give the leg a muscular appearance. I rather prioritize symmetrical implants for female patients. The implant to the lower part of the image below is symmetrical, that is both ends are the same shape.
A set of stickers and the identification card accompany the implants in the box. Stickers representing the right and left implants are placed onto the ID card of the implants and which will further be delivered to the patient after the operation. One set of the stickers is also placed under the surgery note in the patient's medical file. Other stickers remain with the surgeon for use in the necessary medical records.
Symmetrical implants (the curvature of the two ends are the same) provide the leg a softer curve, hence they are rather appropriate for female patients. On the other hand, asymmetric (anatomical) implants (one side is thicker than the other) are rather appropriate for male patients and body building athletes who want a muscular appearance on their legs; they provide the calf a muscular appearance. To the upper part of the image below, there is an example of asymmetric (anatomical) implants and the one at the bottom is an example of symmetrical implants.
Elastomer implants, which consist entirely of solid silicone, can be cut and sculpted for the final shape, if desired. We use these implants if the length of patient's leg and implant are incompatible. Let's say the length of the implant, with the volume we want to use, is longer than the patient's leg. In this case, it is possible to use the elastomer implant by cutting (shortening) at the tip. Elastomer implants, which you can see in the image below, may be simply excised with a bistoury to make suitable for the patient.
The implants in the video below represent exactly 18 years before (18 years before 2013, the date of this video). A patient of mine had heard that crooked legs may be corrected using silicone implants while she was abroad in 1995 and subsequently had applied to a plastic surgeon in İstanbul to undergo this surgery. The surgeon, although a little reluctantly, agreed to perform this surgery. The reason why that surgeon was reluctant to perform this surgery was probably associated with the fact that these operations were not well known in our country among surgeons 18 years ago and moreover it was not easy to import these implants. This surgery was then performed on my patient using implants imported from Spain. However, not only the implants were incorrect (very small), but they were also surgically inserted in the wrong area. My patient could not find a talented surgeon to fix her problem for 16 years. We finally met her, removed these incorrect implants (in 2011) and inserted the implants with the right volume and the right model at the right surgical area. The reason why I shared the video of these implants herein was to exhibit that the implants were still tough when removed after 15 years of initial surgery (these implants are still with me by the time I am writing this text in 2019 and they are still in perfect condition). Our patients often wonder if it is necessary to replace these implants over time. This video is an answer to this question. The implants do not need to be replaced even after many years following the surgery, they may remain where they were placed all through your life. Moreover, the implants we use today are much safer compared to those used 18 years ago. Nor do they cause sagging in the calf or anything like that. That's why you won't need to have surgery again in the future.

This video shows that the silicone gel inside the implants with a high cohesive silicone gel content is not fluid. In one of the cases where I replaced my patient’s prior implant, I scratched the sheath of the removed implant from one end (it was only possible by using scissors) and I have been exhibiting this implant to my patients since 2008. Thus, I provide the patients the opportunity to examine the non-fluid silicone gel content of the implant with their own eyes. On the other hand, it is impossible for the implant inserted in the calf with a surgical intervention to be torn and worn on its own; implants may only be damaged in case a knife is stuck in the leg or the leg is seriously injured during an accident (say, a traffic accident). Even in this case, as you can see, the silicone gel will not disperse. I would like to remind you again that the implant you have watched in the video has been here with us since 2008 and the silicone gel content is still intact and not dispersed.

For sure, we take into consideration the criteria such as the patient's leg length, the size of the implant needed, preoperative width/total leg length ratio while deciding on which brand and which model implant to use.
